I got a blank insurance receipt from 868 merivale spa. Did anyone try to reimburse the money? Need suggestions here

You seem to answer your own question with the last sentence. My advice would be do not commit insurance fraud...

Not sure about your insurance, but in my case I would have to submit the name and licence number of the RMT who performed the service. Unless you got serviced by an RMT I would be very careful about what you submit. Like randomott said, insurance fraud is not something you want to mess with.

It will most likely be rejected, its not fraud if you submit a receipt and its determined to be not an RMT.....fraud is when you submit a receipt for RMT who exists but you never received services from them,you only claim to have and want the dollar value of the 1hr.

I think its crazy to submit a receipt for a place like this, they can easily google the address and then you are in a big trouble with your employer. Not worth the risk at all.

If it was in any way legit, the RMT's Registration # with the College would be included in the paperwork. RMT is regulated under the Regulated Health Professions Act ON. 1991.

FYI it's only fraud on the spa that gave it to you.... meaning if they gave you a receipt it's on them and they will get in shit.... not you because you have the receipt
