Condoms can slip off from too much pre-cum or lube under the condom, not necessarily always about size but a better fit condom is less likely to slip off as well. Easy to keep track of though and easily replaced.
Condoms can slip off for numerous reasons ejaculation, not hard enough, too small . these reasons can also be used for when they break. Now the question is when they break or come off and you don't realize until after the deed is done ; what do you guys do ? Do you apologize or do you say "oh well shit happens" and keep it moving? I know as a lady I would be freaking out if this ever happened. What has been your experience if this has happened to you..

Been having sex for nearly 30 years and at least 80% of that was using condoms. Had 2 break. One when i was a drunken inexperienced teen, and once just out of the blue (tho in retrospect, lack of lube was an issue). Both with GF's. Breakage should be really rare, like really rare. As for slippage, I just have found that the non latex are a little more prone. I think because 1. they are less elastic, so hug you less tightly and 2. They feel more like nothing, so you are less likely to notice slippage as you pull out. Never had it slip off during, only after and this is easy to prevent by grabbing the base on withdrawal. Never had a condom issue with a SP, always with GF's.
Colored and flavored condoms are more prone to break, I don't remember the specifics but the color/flavor coating affects condom strength over time. Would recommend flavored condoms for CBJ but remember to switch to regular condom for FS.
I agree, 5% is high!!! I've never had one break. I've been using Skyn for about a year now and like them.
