It's not entirely clear if he's saying he doesn't know who Atwood is, or whether he's merely saying he wouldn't recognize her.
Seems to me fairly clear he is saying he wouldn't recognize her if he fell over her, and that she has an overly exalted opinion of herself.

Anyone active in Toronto culture, especially in public office, knows exactly who Atwood is and what she looks like. nevertheless, Doug thinks he looks cool pretending he does not know her.

In truth, there is so much that Doug REALLY does not know that it seems a waste of time to fake it when he does.

When Doug Ford gets his Order of Canada then maybe he'll be entitled to talk down to Margaret Atwood. Actually no, he would still rank below her: She is not just a member of the order, she is not even just an officer of the order, she's actually a full fledged Companion of the Order of Canada. The only higher civilian honour in Canada is the Order of Merit, held by only one person in the entire country--and it's not Doug. Until he receives similar honours his lowly status as a mere city councilor would seem to demand he show her a little more respect.

I have no problem with him disagreeing with her view, disagreement is what democracy is all about. But I can't see why on earth he feels the need to show such petty disrespect for someone who has is respected enough to have received one of Canada's highest honours.
