The Smart Serve website is either poorly worded, or incorrect. Identification must have a photo, his/her date of birth and "must reasonably appear to have been issued by a government." The regulations then prescribe specific pieces of ID that are acceptable (the list you quoted), but this does not exclude other options.

As for the Health Card, no one can require you to produce a health card, nor can they record any information from it. However, if you colunteer it, it can be used as ID at the bar's discretion.

See my other response: they cannot be required, nor can any information be recorded. They can, however, be accepted as proof of age if voluntarily submitted. Also, the Act cite was repealed.
