Question: Can an improper saddle cause prostate, back and/or other problems?
Yes , it can cause problems, but you can get more serious damages, if you bike without the saddle. :)

The hole can often cause problems depending on how the seam is managed.
I was told the hole is for ventilation, yes?

Last year a sales associates at a bike shop tried to sell me a Brooks saddle ($130). She said the saddle conforms to the shape of my bum.

Don't take it personally. She meant it conforms to anyone.

Prostate friendly seats are great for road- gravel grinds or Enduro Mtbing because you are in the seat for a long time and it elevates the pressure from your prostate. For typical XC riding where you are constantly sprinting out of the seat those seats are not necessarily important to have but a good bonus. Also the nose of the seat should be pointed down a bit so on the climbs the the seat doesn't push in your prostate when you bend your elbows and bring your body lower to the handlebars. Also your butt will not be sliding back on the climb if your seat is pointed down a bit.
