STI risk is on a spectrum. Anal sex has the highest risk because it's more likely to cause tearing, and most STIs transmit more easily through blood. Vaginal sex is next, and then oral. For the original question - yes, you can get STIs from going down on a woman, but the likelihood depends on the STI. In most cases, it's a lower risk activity.
