As in any sexual activity, there remains the risk on contacting STD's, but is this restricted to SP's only (I think not), mind you, one would think that the risk category would be higher for SP's.
The same can be said of SP's running the risk of contacting STD's from their Clients as well. ( Talk about a High-Risk Occupation)

The key, is protective/safe sexual contact. Condom's at all times would be the norm, as well, limitation of other activities involving the transfer of fluids would be wise. As for a visual "inspection" forget that, I stand to be corrected but the incubation period for STD's can be months in length before visiblel signs are evident, but transmission can still occur.
